|---|---|
| Mode Of Action | Heparin interacts with anti-thrombin III heparin cofactor to produce a substance that immediately neutralizes thrombin by forming a complex with it at its active serine center. It also inhibits platelet function, increases the permeability of vessel walls, inhibits the proliferation of vascular smooth muscle cells, inhibits delayed hypersensitivity reactions, and is involved in the regulation of angiogenesis. |
| Drug Indication | Prophylaxis of post-operation venous thrombosis and embolism in high risk patients. |
| Precautions | Hepatic impairment, renal impairment, not to be used interchangeably with other low molecular weight heparins, patients with induced thrombocytopenia, standard clotting time not useful in monitoring treatment. |
| Contra-Indications | Patients at high risk of haemorrhage, heparin and its derivatives, peptic ulcer, severe hypertension, hypersensitivity to heparin, hepato-renal failure, thrombocytopenia. |
| Dosage | Moderate risk: 20mg, 1-2hours before surgery then 20mg OD for 7-10 days. High risk: 40mg 12hours before surgery then 40mg OD for 7-10 days. |
| Special Information | Low-molecular-weight heparins with an average molecular weight of 4000 to 6500 daltons. They have an attenuated effect on coagulation and prolongation of the activated partial thromboplastin time compared with unfractionated heparin. |
